How research stages are assigned

Stages summarize the study designs available for the identified material in this selection. Human stages require studies in which people received that material; measuring the body’s own peptide or testing a different analogue does not qualify. Controlled studies can measure hormone responses or other biological changes, as well as clinical outcomes. A higher stage can include unsuccessful trials; the adjacent summary explains the findings and limitations.

  1. No direct studies selected. The selected sources do not report direct experiments on this identified compound or blend. Related ingredients may have been studied separately.
  2. Laboratory studies. The selected experiments examine cells, tissue or chemical measurements. No completed animal or human experiment on this identified material was selected.
  3. Animal studies. The selected reports include animal experiments on this identified material. The summary explains whether they measured a biological response, a disease outcome or detection and metabolism.
  4. Early human studies. The selected reports include completed human research without a controlled study. Designs may include case reports, uncontrolled treatment or early pharmacology.
  5. One controlled human study. One completed human study with a comparison group was identified in the selected reports. The summary gives its population, measurement and limitations.
  6. Multiple controlled human studies. At least two separate completed human studies with comparison groups were identified. These can measure physiology, clinical outcomes or unsuccessful treatment; their findings still need individual interpretation.

Multiple reports from one trial count as one study for this classification. The selected-report totals below describe library coverage. Outcome-specific certainty would require a separate assessment of study limitations, consistency, applicability, precision and publication bias.

What it is

Uses and research areas

Semaglutide

FDA-approved semaglutide formulations cover type 2 diabetes, weight management and cardiovascular risk reduction in specified populations. Ozempic injection also covers kidney outcomes in diabetes-associated chronic kidney disease. Wegovy injection has accelerated approval for adults with noncirrhotic MASH, a metabolic fatty-liver disease with inflammation, and moderate-to-advanced scarring. Indications differ between tablets and injections.

Sources

Tirzepatide

FDA-approved tirzepatide formulations cover type 2 diabetes from age ten, long-term weight management in adults with obesity or overweight plus a weight-related condition, and moderate-to-severe obstructive sleep apnea in adults with obesity. Mounjaro also covers cardiovascular risk reduction in adults with type 2 diabetes at high risk.

Research stage

The categories describe the most advanced study designs selected for this library. Each summary explains what was measured and what remains uncertain.

Semaglutide

Multiple controlled human studies

Separate controlled trials established weight loss and fewer cardiovascular events. FLOW found fewer cases of serious kidney deterioration or death from kidney or cardiovascular causes in diabetic kidney disease. In metabolic fatty-liver disease with inflammation (MASH), liver inflammation and scarring improved; follow-up continues. Weight regain followed withdrawal in a selected extension. Oral SOUL recorded numerically fewer combined kidney complications and deaths from kidney or cardiovascular causes than placebo; that difference remained uncertain, while kidney filtration declined more slowly.

Which receptors are activated?

GLP-1 and GIP are hormones released in response to food. Activating their receptors can increase insulin release when blood glucose is high and influence how much a person eats. Both medicines bind albumin, a blood protein that helps them remain in circulation longer.

What did the direct obesity trial show?

SURMOUNT-5 randomly assigned 751 adults with obesity, or overweight with a weight-related complication, who did not have type 2 diabetes to one of the two medicines for 72 weeks. Both were injected under the skin once a week. Participants used the highest trial amount they could tolerate: 10 or 15 mg of tirzepatide, or 1.7 or 2.4 mg of semaglutide. The trial measured the percentage change in body weight; 750 participants received treatment and entered the analyses.

What did the direct diabetes trial measure?

SURPASS-2 randomly assigned 1,879 adults whose type 2 diabetes was inadequately controlled by metformin to weekly injections under the skin for 40 weeks. It compared tirzepatide at 5, 10 or 15 mg with semaglutide at 1 mg. The main measurement was HbA1c, which reflects blood glucose over the preceding months.

What does the labeled human half-life mean?

An elimination half-life describes how long the blood concentration takes to fall by half during the measured elimination phase. It helps explain persistence, but does not tell us that one medicine is more potent or that oral and injected amounts are interchangeable.

Semaglutide

The product label gives an approximate elimination half-life of one week. Absorption differs between tablets and injections.

Is there a randomized cardiovascular comparison between these two?

Preventing heart attacks, strokes or cardiovascular death requires trials that follow those events. The major randomized trials used different comparison treatments and populations, so they do not directly rank semaglutide against tirzepatide for cardiovascular protection.

What is known about safety and tolerability?

Semaglutide

Nausea, vomiting, diarrhea, constipation and abdominal discomfort are common across approved semaglutide products. More serious concerns include pancreatitis, gallbladder disease, dehydration-associated kidney injury and severe gastrointestinal symptoms. Delayed stomach emptying matters around anesthesia because retained stomach contents can enter the lungs. Damage to the retina associated with diabetes, called diabetic retinopathy, requires attention in patients with a history of the condition, especially as glucose control improves. Combining semaglutide with insulin or medicines that stimulate insulin release increases the risk of low blood glucose, called hypoglycemia. These warnings appear in the current Ozempic injection label.

Tirzepatide

Nausea, diarrhea, vomiting, constipation and abdominal discomfort are common. Important risks include severe gastrointestinal symptoms, dehydration-associated kidney injury, gallbladder disease, pancreatitis, serious allergy, and low blood glucose when combined with insulin or a medicine that stimulates insulin release. The Zepbound label includes the risk of stomach contents entering the lungs around anesthesia and an August 2026 warning about damage to the retina addressing temporary worsening with rapid glucose improvement. The current label does not recommend use in severe gastroparesis, in which the stomach empties very slowly.

BPC-157 vs TB-500

BPC-157 has been studied in several animal injury models and a few small human reports. Research on the defined acetylated TB-500 fragment is largely analytical or laboratory work. A small 2026 rat study tested BPC-157 alongside a TB-500-labeled preparation, but the paper did not fully identify that preparation or establish a clear difference between the two compounds.

CJC-1295 with DAC vs modified GRF(1–29)

CJC-1295 with DAC was designed to bind albumin and prolong growth-hormone signaling, and early studies measured its persistence and hormone effects in healthy volunteers. Modified GRF(1–29) shares the modified GHRH core but lacks that extension; most selected reports concern chemical identification, leaving much less human pharmacology to compare.

Semaglutide vs tirzepatide

Direct trials found greater average weight and blood-glucose reductions with tirzepatide at the formulations tested. Receptor biology, available products, approved uses, cardiovascular outcomes and tolerability add context when comparing the two medicines.

Liraglutide vs semaglutide

Liraglutide and semaglutide activate the same GLP-1 receptor. Direct trials found larger average weight and blood-glucose reductions with semaglutide under the protocols tested. The comparison explains those trials, formulation differences, tolerability and separate cardiovascular evidence.

Orforglipron vs semaglutide

ACHIEVE-3 directly compared oral orforglipron capsules with oral semaglutide tablets in adults with type 2 diabetes taking metformin. Orforglipron lowered average blood glucose more. More participants reported stomach or intestinal adverse events or stopped treatment because of adverse events, and the average pulse increase was larger with orforglipron. The results concern the trial formulations and strengths; weight-management products need separate context.
