Epithalon, also called Epitalon, is the four-residue peptide AEDG studied in telomere and circadian biology. Thymosin alpha-1 is a separate N-acetylated 28-residue peptide with immune-cell experiments and controlled clinical trials. Its larger corrected sepsis trial did not establish a mortality reduction. Epitalon’s cellular and limited human-biomarker findings do not supply a human longevity benefit, and the two compounds have no direct clinical comparison in the sources reviewed here.

Defined peptides with distinct relationships to tissue extracts
Epithalon is an alternative spelling of Epitalon, Ala-Glu-Asp-Gly (AEDG). Thymosin alpha-1 is an N-terminally acetylated 28-amino-acid peptide; thymalfasin is its chemically synthesized counterpart. Their length, sequence and molecular biology differ. FDA discusses the free peptide and acetate form separately for both substances, so a common name does not establish the identity of every formulation. FDA (2026), FDA (2024).
The related extracts also need clear identities. Epithalamin is a pineal-gland peptide mixture, while Thymalin is a thymus-derived mixture. The older 266-person report followed these extracts and their combinations for six to eight years. It cannot establish that synthetic AEDG, thymosin alpha-1 or their combination extends human life. Sharing a tissue of origin or a research program does not make these interventions equivalent. Khavinson et al. (2003).
The main differences
Are the two peptides the same as pineal or thymus extracts?
Extract composition, defined peptide sequence and salt form determine which studies belong to each material.
Epitalon
Epithalon/Epitalon is the four-residue sequence AEDG. Its free base and acetate are separate forms. Epithalamin is a pineal extract; the older mortality report of that extract cannot establish a synthetic-AEDG effect.

Thymosin α1
Thymosin alpha-1 is an N-acetylated 28-residue peptide, with thymalfasin its synthetic counterpart. Thymalin is a separate thymus extract. Its older longevity observations cannot be assigned to thymosin alpha-1.

What biological changes did the laboratory studies observe?
Telomere maintenance and immune-cell regulation involve different systems and measurements.
Epitalon
The corrected 2025 study found longer telomeres, the protective structures at chromosome ends, and increased activity of telomerase, an enzyme that maintains those ends, in normal-cell cultures after three weeks relative to untreated controls. Breast-cancer cell lines showed a different maintenance response involving alternative telomere lengthening.

Thymosin α1
Human and murine dendritic-cell experiments linked thymosin alpha-1 to immune priming and regulatory effects involving tryptophan breakdown, the inflammation-restraining signal IL-10 and regulatory T cells that help limit immune responses. The response included control of inflammation rather than uniformly raising immune activity.
What do the selected human reports establish?
Human biomarker changes and survival during sepsis are distinct outcomes; neither supplies a head-to-head anti-aging comparison.
Epitalon
FDA describes forty women with low urinary melatonin-metabolite values assigned to AEDG or saline placebo, with thirty-five additional age-normal reference women. After twenty days, metabolite output rose 1.7-fold with AEDG and remained unchanged with placebo. Sleep outcomes and systematic safety were not reported.

Thymosin α1
The corrected TESTS trial randomized 1,106 adults with sepsis and analyzed 1,089 who received study drug. By day twenty-eight, deaths were 127/542 with thymosin alpha-1 and 132/547 with saline placebo: hazard ratio 0.97 (95% confidence interval 0.76–1.24; P=0.82), leaving a mortality benefit unestablished.

How well is blood exposure described?
A measured human concentration profile is different from a response measured days later in cells or urine.
Epitalon
FDA identified preliminary rabbit fluorescent-label distribution information without demonstrated specificity for intact Epitalon and no additional pharmacokinetic studies. The selected sources provide no validated human blood half-life.
Thymosin α1
In nine healthy volunteers comparing three subcutaneous formulations, serum concentrations peaked after one to two hours and elimination half-life was below three hours. Timosina produced higher exposure than Zadaxin and thymosin alpha-1-HLR, so this small study does not establish equivalence of every product.
What safety and longevity limits are most consequential?
Disease-specific safety follow-up and experimental aging findings cannot establish long-term safety or longevity in healthy adults.
Epitalon
The selected mouse study did not establish an increase in average lifespan, despite longer survival at the tail. The small human biomarker report lacked systematic safety data, and the cancer-cell telomere findings do not quantify cancer risk in people. FDA identifies substantial clinical and product-quality gaps.

Thymosin α1
In TESTS, any adverse event occurred in 66.4% with thymosin alpha-1 and 67.6% with placebo; serious events occurred in 26.8% and 29.3%. These differences remained uncertain during ninety-day follow-up in sepsis. FDA also identifies formulation and immune-reaction concerns for proposed compounded products.

Chromosome-end maintenance and regulation of immune responses
Epitalon’s best-known cellular experiments concern telomeres, the protective structures at chromosome ends, and telomerase, an enzyme that can extend their DNA. The 2003 fibroblast report described induced enzyme activity and longer telomeres. The corrected 2025 study found that normal fibroblast and mammary epithelial cultures lengthened telomeres after three weeks and increased telomerase activity, while breast-cancer cell lines implicated an alternative maintenance pathway after four days. Those observations describe cultured cells rather than a clinical aging outcome. Khavinson et al. (2003), Al-dulaimi et al. (2025).
Thymosin alpha-1 experiments instead examined how immune cells coordinate responses. Romani and colleagues studied dendritic cells, which help organize other immune cells. The peptide primed immune responses and promoted regulatory responses involving IDO, an enzyme that breaks down the amino acid tryptophan, together with IL-10, an inflammation-restraining signal, and regulatory T cells that help limit immune responses. These responses can restrain inflammation as well as support defense. The specific findings are more informative than calling the compound a general immune booster. Romani et al. (2006).
Neither mechanism determines a person’s response on its own. A telomere assay must be linked to a meaningful health outcome, and an immune-cell change must be tested against disease outcomes and harms. The sepsis trials illustrate why that additional clinical step matters.
An earlier survival signal was followed by a larger blinded trial
ETASS enrolled 367 patients with severe sepsis at six Chinese teaching hospitals; 361 remained in the analysis after consent withdrawals. Both groups received conventional care, with thymosin alpha-1 or control injections added. By day twenty-eight, 47 of 181 thymosin alpha-1 recipients and 63 of 180 controls had died (26.0% versus 35.0%). The relative risk was 0.74, with a 95% confidence interval from 0.54 to 1.02 and P=0.062. A time-to-event log-rank test gave P=0.049, while in-hospital deaths were 52/181 versus 71/180. The analysis-dependent pattern supported further investigation without providing a secure general survival conclusion. Wu et al. (2013).
TESTS supplied that later larger test. At twenty-two Chinese centers, 1,106 adults with sepsis were randomized to subcutaneous thymosin alpha-1 or saline placebo alongside care; 1,089 who received study drug entered the main modified intention-to-treat analysis. In the corrected report, 127 of 542 thymosin alpha-1 recipients died by day twenty-eight, compared with 132 of 547 placebo recipients (23.4% versus 24.1%). The hazard ratio was 0.97 (95% confidence interval 0.76–1.24; P=0.82). The interval allows both some benefit and some harm, while the trial did not establish a reduction in mortality. Wu et al. (2025).
The published correction matters because the original survival analysis had not incorporated updated follow-up for nine participants; it also corrected some immune-cell data. The corrected analysis remains the relevant result from the same trial. ETASS and TESTS were separate studies with different designs and populations, and the earlier result should be read alongside the later blinded test. Exploratory subgroup findings do not replace the overall trial result. BMJ (2025).
Epitalon has limited human biomarker data and distinct mouse survival findings
In the small human circadian report, investigators assessed women aged forty to fifty, mainly night-shift workers. FDA’s account identifies seventy-five women overall: thirty-five with age-appropriate urinary melatonin-metabolite values formed a reference group, and forty with low values were assigned to AEDG or saline placebo. After twenty days, the AEDG group’s urinary melatonin-breakdown product rose 1.7-fold, while placebo values did not change; body-clock gene expression also changed. The report did not specify blinding or evaluate sleep outcomes, and did not provide systematic safety data. FDA also noted a claimed general-condition response without a described assessment method, which cannot establish a dependable functional benefit. Ivko et al. (2021), FDA (2026).
The mouse longevity experiment asked a different question. Fifty-four female Swiss-derived SHR mice receiving Epitalon were compared with fifty-four saline controls over their lifetimes. The experiment did not establish an increase in mean lifespan, while mean lifespan among the last tenth of survivors increased 13.3% and maximum lifespan increased 12.3%. Overall spontaneous tumor incidence also did not change, despite a reported reduction in leukemia. These outcomes describe a particular sex and mouse population and do not estimate a human longevity effect. Anisimov et al. (2003).
One peptide has small human formulation studies; the other lacks a validated human curve
Rost and colleagues compared three subcutaneous thymosin alpha-1 formulations in a randomized three-way crossover involving nine healthy volunteers. Serum concentrations measured by an immunoassay peaked at one to two hours, and the reported elimination half-life was below three hours. Repeated short exposure did not show clear accumulation, while Timosina produced higher exposure than Zadaxin and thymosin alpha-1-HLR. This demonstrates why formulation matters even when the peptide name is shared. Rost et al. (1999).
FDA’s Epitalon evaluation identified a preliminary fluorescent-label study in pregnant rabbits, without showing that the detected tissue signal specifically represented intact Epitalon. It identified no additional pharmacokinetic studies in that evaluation. Neither the culture-exposure times nor the human biomarker study supplies a validated human blood half-life. A numerical exposure comparison between Epitalon and thymosin alpha-1 would therefore require an unsupported value for Epitalon. FDA (2026).
The clinical safety observations belong to their populations and products
Within ninety days in TESTS, at least one adverse event occurred in 360/542 thymosin alpha-1 recipients (66.4%) and 370/547 placebo recipients (67.6%). Serious events occurred in 145/542 (26.8%) and 160/547 (29.3%), respectively. Those between-group differences remained uncertain. This is useful observed safety information in critically ill patients, while it does not determine uncommon long-term harms in healthy users or the quality of another formulation. Wu et al. (2025).
Epitalon’s small biomarker report did not supply systematic safety results. Its effects on telomere maintenance in normal and malignant-cell cultures leave a biological safety question without estimating cancer risk in treated people. FDA identifies human safety and product-characterization gaps, including potential immune reactions related to aggregates or peptide impurities. For thymosin alpha-1, FDA similarly distinguishes the studied preparations from proposed compounded products and notes that the agency has approved no drug product containing the peptide; approvals described in other jurisdictions concern specified uses and products. FDA (2026), FDA (2024).
The evidence is therefore unequal in kind and depth. Thymosin alpha-1 has larger controlled disease trials, including a substantial trial that failed to establish its intended mortality benefit. Epitalon has experimental biology and limited human biomarkers without a demonstrated longevity benefit. Those facts help readers understand what has been tested, but they do not identify a superior general anti-aging or immune treatment.
Frequently asked questions
Why does an immune marker not guarantee a survival benefit?
A marker can change without changing the overall course of a complex disease. The earlier ETASS immune and survival signals required clinical confirmation; the larger blinded TESTS trial did not establish lower mortality.
Which TESTS numbers should be used?
The corrected report gives 127/542 versus 132/547 deaths by day twenty-eight and a hazard ratio of 0.97 (95% confidence interval 0.76–1.24). The correction incorporates updated survival follow-up for nine participants and corrects some immune data; it is part of the same trial.
Does the Epitalon melatonin-metabolite result establish better sleep?
The study measured a urinary breakdown product and body-clock gene expression. FDA notes that sleep outcomes were not evaluated and that the reported general-condition improvement lacked a described assessment method. Those observations do not establish insomnia treatment.
Does the older 266-person study establish a benefit of this pair?
It studied Thymalin and Epithalamin extracts, including their combination. Neither synthetic thymosin alpha-1 nor synthetic AEDG can inherit that report’s mortality estimates.
Do foreign approvals or orphan designation establish FDA approval?
They are separate regulatory decisions. FDA’s thymosin alpha-1 evaluation discusses specified foreign authorizations while stating that no drug product containing the peptide had FDA approval. Epitalon’s historical orphan designation was withdrawn; designation itself does not establish efficacy or marketing approval.
