Melanotan II is a synthetic cyclic melanocortin peptide first investigated for pigmentation and later for acute erectile responses. The original human studies involved three volunteers in the pigmentation pilot and ten participants in each of two erectile-function experiments. They demonstrated biological activity, while case reports and laboratory analyses of unregulated products raise consequential safety and quality concerns. A 2026 Australian regulatory investigation adds current evidence that products bearing the name can contain inconsistent amounts of peptide.
Overview
The original research material was a seven-residue, lactam-bridged analogue of α-melanocyte-stimulating hormone. Its sequence is acetylated at the N terminus, contains norleucine and D-phenylalanine, and ends in an amide. These chemical features distinguish the defined molecule from the longer melanotan-I/afamelanotide peptide and the related bremelanotide molecule. Dorr and colleagues (1996).
Molecular profile
Forms and derivatives
Melanotan II commercial salt/formulation labels
Salt, nasal-product or vial labels identify a claimed preparation. Actual peptide content and purity require analytical characterization, which varied in the published sampled products.
Related melanocortin analogues
Afamelanotide/melanotan-I is a longer peptide, and bremelanotide is a distinct cyclic molecule developed in its own clinical program. Their evidence is linked through separate profiles.
The clinical pilot described a particular research preparation. A commercial “Melanotan” name can be ambiguous about the molecule, concentration and additional ingredients. Breindahl's laboratory study addressed that problem directly by testing whether products sold as Melanotan II contained the claimed material and amount. Breindahl and colleagues (2015).
Uses and research areas
The earliest human work asked whether the cyclic peptide could increase skin pigmentation. Spontaneous erections observed during that pilot led to small crossover experiments in men with erectile dysfunction, including separate studies of psychogenic dysfunction and organic risk factors. The investigators measured short-term rigidity and erectile responses during monitored visits. These outcomes characterize acute activity rather than treatment persistence, relationship satisfaction or long-term sexual function. Dorr 1996; Wessells 1998; Wessells 2000.
Commercial tanning products introduce another research issue: what material a person is actually exposed to. Analytical product studies and poisoning reports belong alongside the early controlled preparations because their contents and exposure circumstances can differ substantially. Breindahl and colleagues (2015).
Mechanism of action
Melanotan II activates melanocortin receptors, a family involved in pigment production and other physiological systems. The observed pigmentation, nausea and erectile responses show that its activity can reach several processes. The initial human pilot measured the visible and systemic consequences of peptide exposure; it did not isolate a complete receptor-by-receptor account of those effects. Dorr and colleagues (1996).
Ring formation constrains the molecule's shape, while the D-phenylalanine residue changes its stereochemistry relative to the natural hormone. Structure matters for receptor recognition, but chemical stability alone cannot establish a safe clinical exposure or an advantageous outcome. The later erectile studies provide direct evidence about their measured physiological response, and product analyses show why a sequence name alone cannot define a commercial preparation. Wessells and colleagues (2000); Breindahl and colleagues (2015).
Pharmacokinetics
The acquired primary human reports do not provide a sufficiently resolved Melanotan II plasma analysis to support an exposure curve here. The pigmentation assessment and six-hour rigidity observations measure biological outcomes. Their time windows cannot be substituted for a peptide elimination half-life.
A nasal product also needs its own absorption and exposure measurements. TGA's analysis identified inconsistent peptide amounts in seized nasal bottles, which prevents a product label from serving as a reliable starting point for an assumed exposure calculation. TGA investigation (2026).
Research findings
The original pigmentation pilot
Dorr and colleagues conducted a single-blind study in three healthy men, using alternating peptide and saline exposures during a two-week experiment. Two participants had increased pigmentation on subsequent assessment. Nausea and spontaneous erections were observed, and a higher studied exposure produced moderate somnolence and fatigue in one participant. The pilot supplied initial human activity observations, with a sample too small to estimate consistency or uncommon harms. Original phase 1 report.
Acute erectile-function experiments
A ten-man placebo-controlled crossover study in psychogenic erectile dysfunction reported clinically apparent erections in eight participants. Mean duration of tip rigidity above the specified 80% threshold was 38.0 minutes after Melanotan II versus 3.0 minutes with placebo. A separate ten-man crossover study in men with organic risk factors reported 45.3 versus 1.9 minutes for that rigidity endpoint. These studies involved different cohorts, and the later experiment also included repeated exposures, so participant counts and numbers of exposures require separate denominators. Wessells 1998; Wessells 2000.
Measured contents of products sold online
Breindahl and colleagues analyzed products from three internet shops with validated chromatography and tandem mass spectrometry. Vials claimed to contain 10 mg of Melanotan II contained 4.32–8.84 mg in the sampled products. Unknown impurities represented 4.1–5.9% in samples from two shops. The analysis demonstrated a mismatch between declarations and measured contents without determining the clinical toxicity of each impurity. Its historical selected samples do not describe every contemporary supplier. Original analytical study.
TGA's August 2026 advisory reports a separate investigation of five seized nasal bottles. Estimated total peptide amounts ranged from 22 to 54 mg despite a 30 mg label claim. The different product format and more recent sampling add evidence that preparation variability remains relevant. TGA laboratory investigation.
Safety and tolerability
The small early studies commonly reported nausea, yawning or stretching, and other systemic effects. More serious observations come from clinical cases. Nelson and colleagues described a 39-year-old man with autonomic symptoms, muscle breakdown and kidney dysfunction after internet-purchased material. Creatine kinase reached 17,773 IU/L, and mass spectrometry confirmed Melanotan II against a reference standard. This analytical identification strengthens the material attribution, while a single poisoning case supplies no incidence denominator or general risk estimate. Nelson and colleagues (2012).
A separate case report documents ischemic priapism after reported Melanotan II exposure, requiring surgery after initial management failed. FDA's risk statement also describes published serious-event reports and peptide impurity or aggregation concerns. These observations require careful attention even though case reports cannot determine how often a complication occurs or establish every reported causal link. Mallory and colleagues (2021); FDA risk statement.
Melanotan II has no approved tanning indication identified in this review. TGA explicitly states that no Melanotan II products are approved for supply in Australia. The early clinical experiments and a research-use label do not provide a therapeutic authorization or a verified quality assessment for a marketed product. TGA advisory (2026).
What the research means
The original experiments demonstrate pigment and erectile activity in humans. Their small sizes and acute endpoints leave the practical questions of sustained benefit and repeat-use safety largely unanswered. The later poisoning cases add serious clinical observations, while the product studies show that actual exposure may be uncertain before biological effects are considered.
Taken together, those strands explain why evidence for a defined Melanotan II molecule must remain distinct from assumptions about products sold under its name. The clinical development of related melanocortins offers useful comparisons, with each product retaining its own molecule, indication and safety record.
FAQs
How much human efficacy evidence is there?
The original pigmentation pilot involved three healthy men; each of the two erectile-function crossover studies involved ten men. They establish acute biological activity with limited information about lasting benefit or uncommon harms.
Do erections in the studies imply harmless sexual enhancement?
The studies measured short-term physiological responses. Ischemic priapism has also been reported after exposure and can require operative treatment. Mallory and colleagues.
What did the laboratory analyses reveal?
The 2015 internet-vial study found amounts below the label claim and unknown impurities in some samples. A separate 2026 TGA investigation found inconsistent amounts in five nasal bottles. Each result applies to its sampled products. Breindahl study; TGA advisory.
Can the toxicity case tell us how common severe harm is?
It documents severe illness and analytically identified material in one patient. Estimating frequency needs a denominator and systematic follow-up across exposed people. Nelson case.
