Semax and bremelanotide are both short synthetic peptides related to hormone biology, but their studies ask different questions. Semax reports concern brain signaling and neurological illness; bremelanotide trials concern desire and related distress in premenopausal women with hypoactive sexual desire disorder, meaning persistent low desire that causes distress. No selected trial compares them for memory, stroke recovery or sexual symptoms.

An ACTH-related sequence and a melanocortin agonist have different research aims
Semax is the seven-amino-acid sequence Met-Glu-His-Phe-Pro-Gly-Pro, built from an ACTH-related segment and a Pro-Gly-Pro tail. ACTH is a pituitary hormone, but sequence ancestry does not establish the same hormonal action for the shorter peptide. The rat work examined brain-derived neurotrophic factor, or BDNF, and its receptor TrkB in the hippocampus, a brain region involved in learning. BDNF helps regulate nerve-cell survival and connections; an increase in this protein is a biological measurement rather than a direct measure of human cognition. Dolotov and colleagues (2006).
Bremelanotide, also called PT-141, is a cyclic melanocortin receptor agonist. Its approved acetate injection was studied for acquired, generalized hypoactive sexual desire disorder in premenopausal women. The label identifies receptor activity but states that the exact mechanism of benefit in HSDD remains unknown. Neither a hormone-related sequence nor a general claim of activity in the brain makes the two peptides substitutes for one another.
The main differences
What biological question does each peptide address?
Hormone-related chemistry does not make the clinical uses equivalent.
Semax
Semax is the Met-Glu-His-Phe-Pro-Gly-Pro heptapeptide. Rat studies examined brain-derived neurotrophic factor (BDNF), a protein involved in nerve-cell survival and adaptation, and its receptor TrkB. They also measured learned avoidance behavior. These measurements concern nerve-cell biology rather than sexual desire.

Bremelanotide
PT-141 is bremelanotide, a cyclic melanocortin receptor agonist. The approved acetate injection treats premenopausal women with acquired, generalized hypoactive sexual desire disorder: distressing low desire that developed after previously normal desire and occurs across situations; its exact mechanism of clinical benefit remains unknown.
What did the human studies actually find?
Stroke recovery and desire-related distress were measured in different participants and cannot be placed on one scale.
Semax
The acute-stroke report compared 30 Semax-treated patients with 80 conventional-care controls and described faster recovery, especially of motor deficits. Its accessible abstract does not provide numerical effects or enough allocation detail for a reliable causal estimate.
Bremelanotide
Two 24-week randomized trials found better reported desire and less distress than placebo in premenopausal women with distressing low desire (hypoactive sexual desire disorder). Pooled placebo-adjusted differences were a 0.35-point increase in desire and a 0.33-point reduction in distress; satisfying-event counts did not improve more than placebo.
Does a brain-related result demonstrate better overall performance?
A scan, a questionnaire and functional disease progression are separate measurements.
Semax
A 24-volunteer study detected a larger resting-state brain-network component after Semax, without demonstrating a memory benefit. A separate 27-person motor-neuron-disease report found improved emotional and motivational quality-of-life scores but no influence on the reported denervation or functional course.

Bremelanotide
The controlled findings concern sexual desire and distress. The label excludes general performance enhancement, and the prespecified satisfying-event-count comparison did not establish benefit. Neither the label nor these trials demonstrates better cognition.

Can their human exposure be compared numerically?
Nasal administration, scan timing and measured blood elimination answer different questions.
Semax
The selected human reports and FDA review do not establish an intact-Semax concentration-time profile in people. Scanning at 5 and 20 minutes after a nasal preparation cannot supply a human half-life or a measured brain concentration.

Bremelanotide
After the approved subcutaneous formulation, mean terminal elimination half-life is approximately 2.7 hours (range 1.9–4.0 hours). The label states that the duration of efficacy after an administration is unknown.
What is known about safety and approval?
Known event rates and missing safety information should each be described in their own study setting.
Semax
Neither Semax free base nor acetate is a component of an FDA-approved drug according to the 2026 FDA review. Human harms reporting is incomplete, and FDA identifies potential immune-response and impurity concerns for compounded Semax. These gaps do not establish that it is safer.

Bremelanotide
Vyleesi is approved for premenopausal women with acquired, generalized hypoactive sexual desire disorder, meaning distressing low desire. Nausea occurred in 40.0% versus 1.3% with placebo in the phase 3 safety population; blood-pressure effects and potentially persistent pigment changes are additional warnings.

The Semax stroke reports describe recovery, with substantial design gaps
An older acute-stroke report compared 30 Semax-treated patients with 80 patients receiving conventional therapy after strokes of similar severity and location. The authors described faster regression of neurological problems, particularly motor deficits, with Semax added to intensive care. The accessible abstract does not provide numerical clinical effect sizes, uncertainty intervals or enough allocation and blinding information to establish a reliable causal estimate. Gusev and colleagues (1997).
The result is consequently useful as a reported clinical observation in acute brain injury. It does not tell us whether healthy adults remember more, whether benefit persists with current stroke care, or whether Semax changes sexual desire. Treatment of an injured brain and enhancement of healthy performance would require separate patient populations and outcomes.
Brain imaging and animal proteins are supporting measurements
In 24 healthy volunteers, 14 received nasal Semax and 10 received placebo. Resting-state functional MRI was performed before exposure and 5 and 20 minutes afterward. Researchers detected a larger medial-frontal component of the default-mode network in the Semax group. This network is a pattern of correlated activity seen while a person is resting. A larger detected component does not mean the anatomical brain grew, and the experiment did not demonstrate better memory, concentration or daily function. Lebedeva and colleagues (2018).
In the separate rat study, nasal Semax was associated with a maximum 1.4-fold increase in hippocampal BDNF protein and a 1.6-fold increase in TrkB phosphorylation, a chemical change associated with receptor activation. The animals also made more conditioned avoidance responses. This supplies a possible connection between signaling and learned behavior in rats, while leaving a human cognitive benefit to be measured directly. Dolotov and colleagues (2006).
The motor-neuron-disease report separates quality of life from function
A 27-person Semax report in motor-neuron disease described an open-label study in sequential groups, despite being indexed as a randomized controlled trial. The authors reported no influence on the course of partial denervation or the clinical functional estimates. Denervation means that muscle fibers have lost their nerve supply; needle electrical recordings helped follow that process. The abstract does not provide a numerical effect estimate or a clearly defined concurrent control. Serdiuk and colleagues (2007).
Overall quality-of-life scores improved through emotional state and motivation, with the largest reported response on day 10. This subjective observation can matter to patients while remaining a different outcome from slowing functional deterioration. The report therefore does not support a general conclusion that every neurological measurement improved with Semax.
Bremelanotide has two controlled HSDD trials, with narrower conclusions than performance enhancement
The two 24-week RECONNECT trials randomized 1,267 premenopausal women; 1,247 entered the safety population and 1,202 the main efficacy analysis. Women had low desire that developed after previously normal desire, occurred across situations and caused distress. Compared with placebo, pooled desire scores improved by 0.35 points and distress about low desire fell by 0.33 points, both P<0.001. The number of satisfying sexual events did not increase more than with placebo (P=0.630). Kingsberg and colleagues (2019).
These replicated symptom results support a defined indication, rather than a broad claim of improved sexual performance. More women withdrew from bremelanotide than placebo, and missing outcomes were analyzed by carrying the last observation forward. The longer open-label follow-up involved the same cohorts; it adds observation time without becoming another independent placebo-controlled test. Semax’s stroke, imaging and motor-neuron-disease reports did not measure these HSDD outcomes.
The route and tested formulation define what is known about exposure and harm
The Vyleesi label reports a mean terminal elimination half-life of about 2.7 hours after subcutaneous administration, with a range of 1.9–4.0 hours. The selected Semax clinical reports and FDA review do not establish a comparable human intact-peptide concentration-time study. A response 5 or 20 minutes after nasal administration is a measurement schedule, not a half-life or proof of a particular brain concentration. The FDA review also notes inconsistent form reporting and distinguishes the free peptide from acetate and modified products sold under related names.
Nausea occurred in 40.0% of bremelanotide recipients versus 1.3% with placebo in the phase 3 safety population. Vyleesi can temporarily raise blood pressure and slow heart rate, and it can produce pigment changes that may persist. Uncontrolled hypertension and known cardiovascular disease are contraindications. These risks are documented for the medicinal formulation; the label also identifies effects on stomach emptying and oral naltrexone exposure.
Semax has less complete clinical harms reporting in the selected publications. FDA’s 2026 staff review states that neither Semax free base nor Semax acetate is a component of an FDA-approved drug and describes missing human safety information. Its compounding-risk page identifies possible immune responses related to aggregation and peptide impurities. These are safety concerns and information gaps, without a comparative event rate against bremelanotide. The staff briefing is an evaluation for compounding deliberations, not approval of a drug.
No common clinical outcome connects these treatments
Bremelanotide has the clearer controlled evidence for the specific HSDD population studied. Semax’s published observations concern other settings, with varying designs and incomplete access to clinical methods. A more developed trial program for one indication does not show that bremelanotide treats neurological illness, and Semax brain activity does not establish an effect on sexual symptoms. A useful comparison therefore asks what each study measured and whether that outcome matches the proposed use.
Frequently asked questions
Does shared ACTH or melanocortin terminology make the peptides interchangeable?
The sequences, pharmacology, routes and studied outcomes differ. Semax brain signaling and bremelanotide HSDD symptom results do not supply a shared clinical effect.
Did the Semax MRI study show brain growth?
It measured the spatial extent of a detected functional network component, rather than growth of anatomical brain tissue. It also did not demonstrate better memory or daily function.
Did the motor-neuron-disease study show slower progression?
The authors reported no influence on the course of partial denervation or the clinical functional estimates, while describing improved emotional and motivational quality-of-life scores. Its open-label methods and missing numerical estimates limit causal interpretation.
Does FDA consideration for compounding equal drug approval?
The 2026 Semax briefing evaluates substances proposed for compounding. It states that neither free base nor acetate is a component of an FDA-approved drug; the briefing itself is not a marketing approval.
