Ipamorelin and GHRP-2 stimulate the ghrelin-receptor system. Their research differs: ipamorelin’s often-cited selectivity result comes from animals, and its human bowel-surgery trial left clinical benefit uncertain. GHRP-2 increased how much people ate in small controlled meal experiments. These findings do not establish a ranking for muscle growth, injury recovery or long-term safety.

Two peptides that stimulate the ghrelin-receptor system
Ipamorelin and growth hormone-releasing peptide 2 (GHRP-2) stimulate a receptor system also activated by ghrelin, a hormone involved in growth hormone release and appetite. They are distinct synthetic peptides, with their own exposure, hormone and clinical studies. Raising growth hormone is a physiological effect whose clinical meaning depends on the population and measured outcome.
Ipamorelin is a five-residue peptide in the original pharmacology paper. Its reputation for selectivity comes from comparisons in cells, rats and pigs. The human evidence then moves to healthy-men exposure experiments and postoperative gut recovery. GHRP-2’s directly relevant controlled human experiments examined appetite and the amount eaten during a laboratory meal. Those different questions explain why one study’s apparently larger response cannot establish the better treatment for another purpose.
The main differences
What was the main biological distinction in the original comparisons?
Both peptides stimulate the ghrelin-receptor system. The original selectivity experiment compared hormone responses in pigs, using growth hormone-releasing hormone as a reference. ACTH signals the adrenal glands to release cortisol, a stress hormone.
Ipamorelin
In pigs, ipamorelin released growth hormone, while ACTH and cortisol responses were not established as higher than with growth hormone-releasing hormone. This is a defined animal hormone result, without demonstrated absence of human side effects.
GHRP-2
GHRP-2 increased ACTH and cortisol in the tested pig conditions. ACTH signals the adrenal glands to release cortisol, a stress hormone. The experiment did not establish a human safety ranking against ipamorelin.
What did the controlled human experiments actually measure?
The trials asked different questions: patients’ return to meal tolerance after bowel surgery for ipamorelin, and food chosen at a laboratory buffet for GHRP-2.
Ipamorelin
Among 114 people in the main postoperative analysis populations, median time to tolerate a solid meal was 25.3 hours with intravenous ipamorelin and 32.6 with placebo. The favorable numerical difference did not establish benefit (P=0.15).
GHRP-2
Seven lean healthy men ate 35.9% more on GHRP-2 infusion visits than on saline visits, and every man ate more (P=0.008). This measured appetite-related food intake, without demonstrating sustained weight or muscle gain.
Does the research establish which increases appetite or muscle growth more?
The studies did not randomize people between ipamorelin and GHRP-2 for appetite, body composition or strength. A shared receptor cannot supply that missing experiment.
Ipamorelin
The human studies measured peptide exposure, growth hormone release and postoperative meal tolerance. They did not establish a comparative appetite increase, sustained muscle growth or superior recovery from injury.

GHRP-2
In a nineteen-person crossover meal study, food intake rose 10.2% and 33.5% in two GHRP-2 exposure conditions relative to placebo. This confirms an acute feeding response in that setting without establishing superiority to ipamorelin or sustained muscle gain.
What can be said about human half-life?
The ipamorelin study measured the infused peptide in blood and the growth hormone released in response. The GHRP-2 meal studies measured food intake and hormones during subcutaneous infusion, without supplying a comparable peptide-disappearance estimate.
Ipamorelin
After short intravenous infusions in healthy men, measured ipamorelin had a terminal half-life of about two hours. Growth hormone peaked around 0.67 hours. These data do not define the absorption of every subcutaneous preparation.
GHRP-2
The selected human meal studies used subcutaneous infusions and measured food intake and hormones. They do not supply a peptide half-life directly comparable with intravenous ipamorelin. Their visit duration is not an elimination estimate.

Can animal selectivity or short trials establish which is safer?
No cited head-to-head clinical safety trial resolves the comparison. Serious events also arose in very different clinical populations, making causality and background illness important.
Ipamorelin
Animal hormone selectivity does not establish human safety. FDA reviews two postoperative deaths with uncertain drug attribution and identifies immune-reaction and quality concerns, with insufficient safety information for some other injectable routes.

GHRP-2

What the animal hormone comparison found
Raun and colleagues (1998) compared ipamorelin with other hormone-releasing signals in experimental systems. In pigs, ipamorelin increased growth hormone, while its ACTH and cortisol responses were not established as higher than those with growth hormone-releasing hormone (GHRH). ACTH is a pituitary signal to the adrenal glands; cortisol is one hormone those glands release during the stress response. GHRP-2 and GHRP-6 increased ACTH and cortisol under their tested conditions.
That experiment supports a particular hormone-selectivity distinction in pigs. It did not measure human adverse-event rates, years of glucose control or outcomes in people using either peptide for athletic recovery. The measured hormone pattern describes these pig experiments, with human adverse-event rates and long-term comparative safety unresolved.
Ipamorelin’s human study moved from hormone release to meal tolerance after surgery
Gobburu and colleagues (1999) studied healthy men and measured ipamorelin and growth hormone after short intravenous infusions, with eight men at each of five exposure levels. The abstract does not establish how many unique men participated across the levels. Released growth hormone peaked around 0.67 hours, while the peptide’s estimated terminal half-life was about two hours. These are separate measurements: hormone release follows peptide exposure without being identical to it.
Beck and colleagues (2014) conducted the later clinical trial, which enrolled 117 adults after small- or large-bowel surgery; 114 formed the main safety and efficacy analysis populations. Investigators compared intravenous ipamorelin with placebo and measured time until patients tolerated a standardized solid meal. Median time was 25.3 hours with ipamorelin and 32.6 hours with placebo. The earlier numerical time was too uncertain to establish a treatment effect (P=0.15), and secondary analyses also did not establish differences. The study leaves a possible benefit unresolved, rather than proving that outcomes were equal.
GHRP-2 increased food intake in controlled human experiments
Laferrère and colleagues (2005) studied seven lean healthy men on separate GHRP-2 and saline-infusion visits. After infusion under the skin, each man ate more at a buffet during the GHRP-2 condition. Mean intake increased 35.9% (P=0.008). Growth hormone rose as well, while the proportions of fat, carbohydrate and protein eaten did not establish a difference. Saline provided a control infusion without the peptide.
Their later randomized crossover experiment (2006) studied ten lean and nine obese, otherwise healthy, weight-stable adults over three visits. Compared with placebo, food intake increased 10.2% in the lower-exposure GHRP-2 condition and 33.5% in the higher-exposure condition. The study did not establish a different response according to obesity status. These are acute meal measurements. They do not demonstrate sustained weight gain, successful nutritional rehabilitation or muscle gain.
The GHRP-2 participants chose how much to eat at a laboratory meal. Ipamorelin’s postoperative patients were recovering bowel function after surgery. Appetite, the amount chosen and the ability to tolerate food are different outcomes, so these experiments cannot establish which peptide improves postoperative recovery or appetite more.
Human exposure cannot be inferred from a hormone peak or rat study
Ipamorelin’s approximately two-hour terminal half-life describes the measured peptide after intravenous infusion in healthy men. Its growth hormone peak around 0.67 hours describes a different substance released by the pituitary. Neither value establishes exposure from every subcutaneous product or the duration of a clinical benefit.
The selected GHRP-2 meal studies used controlled subcutaneous infusions and measured appetite, food intake and hormones. They do not supply a directly comparable human peptide half-life. The duration of a meal experiment does not show how quickly the peptide leaves human blood, and rat clearance findings concern a different species.
Different safety observations do not identify a safer long-term option
Adverse events after treatment began occurred in 87.5% of ipamorelin recipients and 94.8% of placebo recipients in the postoperative trial. FDA’s detailed review discusses two deaths among ipamorelin recipients with major postoperative complications; whether ipamorelin contributed was unclear. It also identifies higher rates of some blood-glucose and potassium abnormalities and missing safety data for the proposed subcutaneous route. The relatively common background events after surgery make raw percentages unsuitable as a safety ranking.
For GHRP-2, FDA identifies reports including increased insulin requirements, infection, pancreatitis and deaths in critically ill patients, with causality unestablished. For injectable or nasal GHRP-2 and some ipamorelin routes, the agency also describes possible immune reactions associated with aggregation and peptide impurities. Small meal studies and animal hormone selectivity do not resolve these clinical or product-quality risks.
FDA’s compounding guidance explains that a compounded drug is not FDA approved and does not receive the agency’s premarket review of safety, effectiveness and quality. No cited experiment compares long-term benefit or safety between these two peptides for muscle growth, healing or healthy aging. The ipamorelin profile and GHRP-2 profile give the individual study settings.
Frequently asked questions
Does “selective” mean ipamorelin has no side effects?
The original comparison concerned selected hormone responses in pigs. It did not measure all human harms, and later postoperative safety observations leave substantial questions.
Did ipamorelin prove ineffective after surgery?
Patients given intravenous ipamorelin tolerated a standardized solid meal at a median 25.3 hours versus 32.6 hours with placebo. The earlier numerical time did not establish a treatment benefit (P=0.15). The result leaves benefit uncertain rather than proving equal effects.
Did GHRP-2 make every participant eat more?
In the seven-man saline-controlled experiment, every participant ate more during GHRP-2 exposure. The sample was small and the observation concerned one laboratory meal, without a long-term weight outcome.
Do these studies validate combining the peptides?
The studies tested distinct preparations and outcomes. They do not establish that combining two ghrelin-receptor stimulants improves benefit or avoids harm.

Ipamorelin released growth hormone in cells, rats and pigs. In pigs, responses of ACTH, a pituitary signal to the adrenal glands, and the stress hormone cortisol were not established as higher than responses to growth hormone-releasing hormone (GHRH). GHRP-2 and GHRP-6 raised those hormones under the tested conditions.
