Ipamorelin vs sermorelin

Ipamorelin acts through the ghrelin-receptor system. Sermorelin is the amidated fragment containing the first twenty-nine residues of growth hormone-releasing hormone (GHRH) and stimulates its receptor. Ipamorelin’s postoperative trial left meal-tolerance benefit uncertain. Historical sermorelin research measured growth in children deficient in growth hormone (GH) and body composition in a small HIV study. Neither literature establishes superior healthy-adult muscle growth, recovery or anti-aging effects.

Two receptor inputs and a specific historical product

Ipamorelin is a synthetic five-residue peptide in the ghrelin-receptor system. Sermorelin is the amidated first twenty-nine residues of growth hormone-releasing hormone (GHRH). It stimulates the GHRH receptor on pituitary cells, which release GH. Stimulating these different receptor systems can converge on the same hormone without establishing the same response or clinical benefit.

Sermorelin was the ingredient in the historical Geref products. It differs from the four-substitution peptide modified GRF(1–29) and from albumin-binding CJC-1295 with DAC. A pharmacokinetic result from one of those preparations cannot be assigned to sermorelin simply because all three derive from GHRH. The exact fragment and formulation matter when reading the human studies.

The main differences

What receptor and exact peptide does each name identify?

Both can stimulate growth hormone (GH) release, but their structures and receptor inputs differ. Sermorelin also needs separating from other GHRH-derived preparations.

What clinical outcomes did the selected trials measure?

The ipamorelin study examined postoperative meal tolerance; the selected pediatric sermorelin study examined growth speed in diagnosed GH deficiency. They do not establish relative efficacy in one condition.

Ipamorelin

In a bowel-resection trial enrolling 117 adults, 114 entered the main analysis populations. Median time to tolerate a solid meal was 25.3 hours with intravenous ipamorelin and 32.6 with placebo; the earlier numerical time did not establish benefit (P=0.15).

Is there human evidence of a change in adult body composition?

Lean mass, trunk fat, visceral fat and strength are different outcomes. A disease-specific body-composition result cannot establish healthy-adult muscle growth.

Sermorelin

In 31 randomized men with HIV lipodystrophy, an abnormal distribution of body fat, Geref increased lean mass by 0.9 kg versus a 0.3-kg decline with placebo (P=0.04). Visceral fat, the fat around abdominal organs, decreased 19.2 cm² versus a 2.3-cm² increase, but that comparison remained uncertain (P=0.07). The twelve-week study did not establish a strength benefit.

What do the intravenous blood-disappearance studies show?

These estimates concern the administered peptides under particular sampling and assay conditions, rather than the duration of every downstream hormone effect.

Ipamorelin

Short intravenous infusions in healthy men produced an estimated terminal peptide half-life of about two hours. Growth hormone peaked separately around 0.67 hours, showing that peptide exposure and hormone release have distinct time courses.

Sources

Sermorelin

In ten men receiving intravenous infusions, natural amidated GHRH(1–29) had a disappearance half-time of 4.3 ± 1.4 minutes (means ± standard errors). A singly substituted analogue lasted 6.7 ± 0.5 minutes; that analogue’s value must not be assigned to sermorelin or every modified GRF product.

How should approval history and safety be interpreted?

A historical product approval, a withdrawal determination and current compounding quality are separate facts. None supplies a direct long-term safety comparison.

Sermorelin

Historical Geref products were discontinued and approvals withdrawn in 2009; FDA determined in 2013 that withdrawal from sale was not for safety or effectiveness reasons. Current compounded products do not inherit FDA approval from that history, and no long-term safety advantage over ipamorelin follows.

Ipamorelin: blood exposure and a postoperative test

The healthy-volunteer study measured ipamorelin and GH after short intravenous infusions. It included eight men at each of five exposure levels, without reporting the total number of unique participants in the abstract. Estimated peptide terminal half-life was about two hours, while GH peaked in one episode around 0.67 hours. These measurements establish an acute hormone response, without a tested effect on strength or injury recovery.

The postoperative trial enrolled 117 adults after bowel resection and included 114 in its main safety and efficacy populations. Median time to tolerate a standardized solid meal was 25.3 hours with intravenous ipamorelin and 32.6 hours with placebo. The earlier numerical time did not establish benefit (P=0.15), and the secondary analyses also did not establish differences. The uncertainty leaves a possible effect unresolved; it does not establish equal treatment effects.

Sermorelin studies measured growth and disease-specific body composition

Thorner and colleagues treated 110 previously untreated prepubertal children with GH deficiency in a multicenter open-label study; 86 were eligible for efficacy analysis. With subcutaneous GHRH(1–29), mean height velocity, the yearly rate of growing taller, rose from 4.1 ± 0.9 cm/year before treatment to 8.0 ± 1.5 at six months and 7.2 ± 1.3 at twelve months. Without a concurrent placebo group, the result is a before-and-during-treatment observation in selected children. It does not establish final adult height or an effect in adults whose growth plates have closed.

Sermorelin also has a small adult clinical study. Koutkia and colleagues randomized 31 men with HIV-related lipodystrophy, a disturbance of body-fat distribution, to subcutaneous Geref or placebo for twelve weeks; 29 completed. The primary outcome, insulin-like growth factor 1 (IGF-1), a protein involved in growth-hormone signaling, rose by 104 ng/mL with Geref and 6 with placebo (P=0.004). Lean mass increased 0.9 kg versus a 0.3-kg decline (P=0.04), and trunk fat decreased 0.4 kg versus a 0.2-kg increase (P=0.03). Lean mass measured by scanning includes nonfat tissues and is not a direct strength measurement.

Fat around the abdominal organs, called visceral fat, decreased by 19.2 cm² with Geref and increased by 2.3 cm² with placebo. This favorable numerical difference remained uncertain (P=0.07), while the lean-mass and trunk-fat differences were more clearly established. The trial involved HIV lipodystrophy and did not compare Geref with ipamorelin or establish general obesity treatment.

The measured fragments disappear on different time scales

Soule and colleagues infused ten healthy men intravenously with the natural amidated GHRH(1–29) fragment and a version containing one D-Ala2 substitution. The natural fragment, which has the sermorelin sequence, had a disappearance half-time of 4.3 ± 1.4 minutes. The single-substitution analogue had a half-time of 6.7 ± 0.5 minutes (means ± standard errors; P<0.05). This is a comparison between two precisely described peptides, not a concentration study of every modified GRF product.

Ipamorelin’s estimated two-hour terminal half-life came from a different intravenous infusion experiment. The studies support different observed peptide-persistence time scales, while differences in assays and study designs limit a direct quantitative comparison. Neither blood half-time defines how long downstream GH or IGF-1 acts, and neither supplies the exposure or safety of a combined subcutaneous product.

Geref’s history and the limits of current safety claims

Geref had separate historical indications for evaluating pituitary GH secretion and treating idiopathic GH deficiency in children with growth failure. Its products were discontinued and their approvals withdrawn effective June 2009. In 2013, FDA determined that withdrawal from sale was not for reasons of safety or effectiveness. That finding preserves the actual history; it neither means that Geref was banned for toxicity nor gives every current compounded sermorelin product FDA approval.

The open-label childhood study reported no adverse changes in monitored biochemical or hormonal measurements. The small HIV study did not establish significant between-group changes in glucose, insulin or lipids over twelve weeks. Such observations remain tied to their populations, preparations and follow-up. FDA explains separately that compounded products do not undergo its premarket approval review for safety, effectiveness and quality.

Ipamorelin’s original pig hormone-selectivity experiment does not establish human safety. FDA identifies immune-reaction and peptide-quality concerns and discusses serious events, including two deaths with unclear drug attribution, in postoperative intravenous research. A direct randomized comparison of the two peptides’ long-term harms was not identified in the cited literature.

How to compare the findings without extending them

Sermorelin has historical pediatric clinical development and a small randomized adult body-composition study, while ipamorelin has a human exposure study and an uncertain postoperative clinical result. The actual findings differ in population and outcome. They do not establish which is more effective for healthy-adult muscle gain, recovery, sleep or longevity, and they do not validate a combination of the two.

The sermorelin profile covers its broader childhood and adult literature, and the ipamorelin profile explains the postoperative trial. For the other GHRH preparations, read modified GRF(1–29) and CJC-1295 with DAC vs ipamorelin. Their molecular changes determine which exposure results apply.

Frequently asked questions

Was Geref withdrawn because it was unsafe?

FDA’s 2013 determination says the historical products were not withdrawn from sale for safety or effectiveness reasons. They had been discontinued, and their approvals were withdrawn in 2009. Those facts do not establish approval of current compounded products.

Does the HIV study prove a general bodybuilding effect?

The small Geref trial involved men with HIV lipodystrophy. Scanning measured lean mass and fat distribution over twelve weeks; lean mass is not a direct measure of strength, and the study did not establish healthy-adult bodybuilding benefits.

Did Geref have no effect on visceral fat?

The measured change favored Geref, with a 19.2-cm² reduction versus a 2.3-cm² increase with placebo. P=0.07 left that comparison too uncertain to establish benefit; it does not demonstrate equal effects.

Does receptor complementarity validate combining sermorelin and ipamorelin?

Different receptor inputs explain a biological rationale, while the cited individual-agent trials do not establish the effectiveness or safety of their combination. FDA’s ipamorelin review also notes a combination-product adverse-event report that could not isolate one ingredient’s contribution.
