BPC-157 has been studied in several animal injury models and a few small human reports. Research on the defined acetylated TB-500 fragment is largely analytical or laboratory work. A small 2026 rat study tested BPC-157 alongside a TB-500-labeled preparation, but the paper did not fully identify that preparation or establish a clear difference between the two compounds.

The materials determine which papers apply
BPC-157 and TB-500 appear together in discussions of tissue repair because their literature touches cell movement, injury and the organization of healing tissue. They have different chemical identities and development histories. BPC-157 is a synthetic 15-residue peptide with the sequence GEPPPGKPADDAGLV. FDA’s 2026 BPC-157 assessment distinguishes the free base from its acetate form, a distinction relevant to identifying the preparation in a report.
For TB-500, the most useful starting point is a defined sequence: Ac-LKKTETQ, an N-terminally acetylated seven-residue fragment corresponding to positions 17–23 of thymosin beta-4. Esposito and colleagues identified that fragment in a TB-500-labeled product in 2012. FDA’s 2026 assessment evaluates this fragment and its acetate form. Full-length thymosin beta-4 contains 43 residues; unacetylated LKKTETQ is another preparation. A result obtained with one of these peptides cannot establish what another one will do.
This identity work becomes especially consequential in the direct tendon comparison. Its authors used a preparation labeled TB-500 and supplied a product code, but the paper provides no sequence or independent structural characterization. Without the sequence, we cannot tell whether the product was the defined acetylated fragment, so its tendon result cannot be confidently assigned to that fragment.
The main differences
Which material does the name describe here?
BPC-157
Synthetic 15-residue GEPPPGKPADDAGLV; free-base and acetate identities are distinguished.
TB-500
Defined TB-500 is Ac-LKKTETQ, an acetylated seven-residue fragment. The 2026 tendon paper did not report the peptide sequence of the product it called TB-500.

What happened to tendon strength in the rat study?
Researchers cut and surgically repaired the Achilles tendons of 32 rats, then assigned eight rats each to BPC-157, the product identified as TB-500 in the study, both peptides or neither peptide. The rats in the last group received the injection liquid without either peptide; the paper calls this liquid the vehicle but does not specify its composition. Thirty days after surgery, the investigators pulled four repaired tendons from each group until they broke and recorded the force they could withstand.
BPC-157
The tendons from rats given BPC-157 withstood more force than those from rats given neither peptide. The difference was too uncertain to establish an effect of BPC-157 in this small experiment (adjusted P=0.389).
TB-500
The tendons from rats given TB-500 also withstood more force than those from rats given neither peptide, and this difference met the study's threshold for statistical significance (adjusted P=0.0406). The study did not establish that TB-500 worked better than BPC-157.
What human recovery evidence is available?
BPC-157
Small uncontrolled knee-pain reports include patient-reported improvement after local treatment; they establish no controlled tendon-healing effect.
TB-500
FDA found no clinical studies of the evaluated fragment. Full-length thymosin beta-4 human studies concern a different material.

Does the combination have demonstrated added benefit?
BPC-157
In the 2026 rat study, combining BPC-157 with the product identified as TB-500 did not establish greater repaired-tendon strength than giving either peptide alone. Only four tendons per group were tested.
TB-500
The rat study did not establish an added strength benefit from combining the peptides. Neither that experiment nor the small uncontrolled human knee series establishes that the combination works better in people.

From cell behavior to repaired tendon strength
What the earlier experiments measured
Tendon repair requires cells to reach the injury, remain viable and organize a collagen-rich matrix that can carry force. Chang and colleagues (2011) studied BPC-157 in rat Achilles-tendon explants and isolated tendon fibroblasts. They observed greater outgrowth, migration, spreading and survival under oxidative stress, accompanied by changes in actin and attachment-associated FAK/paxillin signaling. Their assays found no direct increase in fibroblast proliferation. This helps specify the observed response: the experiments measured several components of cell movement and survival, with a different result for direct proliferation.
Earlier whole-animal work by Staresinic and colleagues (2003) followed rats after complete, unrepaired Achilles-tendon transection. Compared with saline, intraperitoneal BPC-157 improved mechanical, walking-related and tissue-structure measurements over assessments through day 14. That model began with a gap between the cut tendon ends. It differs from a tendon immediately repaired by a surgeon and tested later, which matters when interpreting the newer result.
Defined acetylated TB-500 has a smaller experimental record. Rahaman and colleagues (2024) examined the fragment and its metabolites in laboratory systems, rat urine and a fibroblast scratch assay. The investigators scratched a gap into a layer of cultured fibroblasts and measured how much the gap closed. Cells exposed to the breakdown product Ac-LKKTE closed the gap more than control cells, with a statistically significant difference. The comparison using the original TB-500 fragment did not establish the same effect. These results distinguish the fragment from one of its breakdown products in this cell assay.
The adjacent thymosin literature helps explain the interest but introduces additional materials. Philp and colleagues (2003) studied full-length thymosin beta-4 in diabetic and aged mice with skin wounds, and an unacetylated LKKTETQ fragment in aged animals. Full-length thymosin beta-4 increased wound contraction and collagen deposition in the diabetic and aged mice; the unacetylated fragment also promoted repair in aged mice. Skin closure in these animals helps explain the interest in tendon repair, but the tested peptide, tissue and injury differ from acetylated TB-500 in a surgically repaired Achilles tendon.
The direct 2026 comparison
Biçer and colleagues (2026) cut and immediately repaired the Achilles tendons of 32 young male Sprague-Dawley rats. They randomly assigned eight rats each to BPC-157, the product identified as TB-500 in the study, both peptides or neither peptide. The last group received the injection liquid without either peptide; the authors call this liquid the vehicle but do not specify its composition. The rats received daily injections into the abdominal cavity through day 30. At that point, four tendons from each group were pulled until they broke, while the other four were examined for tissue changes.
The repaired tendons from rats given BPC-157 or TB-500 withstood more pulling force before breaking than those from rats given neither peptide. This force is the maximum load to failure. Only the TB-500 comparison with the group given neither peptide met the threshold for statistical significance after adjustment for multiple comparisons (P=0.0406); the corresponding BPC-157 comparison gave P=0.389. The study did not establish a difference in this measurement between the two peptides or an added benefit from combining them. Selected tissue scores also showed better organization in the TB-500 group than in the group given neither peptide.
A statistically significant result for one peptide against the group given neither peptide, alongside an uncertain result for the other, does not establish that the first peptide works better. That requires a comparison between the peptides themselves. Here, only four tendons per group were tested, all at one time point; the investigators did not measure walking or other functions. The paper also leaves the TB-500 product’s peptide sequence unreported. A larger study with later follow-up and functional measurements would be needed to understand how these findings relate to recovery.
How the rat study tested repaired tendons
Thirty-two rats had their Achilles tendons cut and surgically repaired, then received BPC-157, TB-500, both peptides or neither peptide. Four tendons per group were pulled until they broke on day 30, and the other four were examined under the microscope.

- Selected scores were better with TB-500 than with neither peptide
- The experiment did not measure walking or recovery in people
Human symptom reports answer narrower questions
The most frequently cited musculoskeletal human report is Lee and Padgett’s 2021 retrospective knee-pain series. Seventeen records were reviewed and sixteen patients were reached by telephone, usually six to twelve months after treatment injected into the knee joint. Eleven of twelve BPC-157-only recipients reported improvement, alongside three of four recipients of BPC-157 plus a preparation described as thymosin beta-4. The small groups were formed by treatment history, with no placebo arm, randomized assignment or standardized test of knee function.
Pain relief is meaningful to a patient, but that survey could not separate a peptide effect from other influences or establish tissue repair. The combination group also cannot establish whether adding a defined TB-500 fragment helps: its reported TB4 identity and uncontrolled four-person design leave both the material comparison and attribution unresolved. The 2026 rat combination result and this human series concern different materials, injuries and endpoints.
FDA’s BPC-157 briefing also discusses older digestive studies, including an ulcerative-colitis conference abstract whose complete trial report was unavailable. Its TB-500 assessment did not identify clinical studies of the evaluated fragment in humans. Human trials of full-length thymosin beta-4 exist, including Sosne, Dunn and Kim’s small eye-drop trial in nine people with severe dry eye, with 28 days of treatment and another 28 days of observation. Changes in eye symptoms and corneal staining after full-length thymosin beta-4 eye drops cannot establish whether injected seven-residue TB-500 improves recovery from a tendon injury.
What the blood and metabolism studies measured
A repair experiment measures the tissue’s response; pharmacokinetics measures what material reaches the body and how it changes over time. He and colleagues (2022) measured intact BPC-157 after intravenous and intramuscular administration in rats and beagle dogs. They separately followed radioactivity derived from labeled peptide. Breakdown products can retain a radioactive label after the original peptide has disappeared, so tissue radioactivity and intact blood peptide describe different analytes.
The TB-500 metabolism experiments similarly distinguish the parent fragment from detected metabolites. A urinary breakdown product is evidence about detection and metabolism; its persistence supplies no numerical human blood half-life. Neither the BPC-157 animal study nor the TB-500 analytical study supplies a matched human exposure comparison. A joint concentration-time graphic would therefore imply measurements the cited research has not established.
What remains uncertain about benefit and safety
Clinical safety remains poorly characterized for both materials. Lee and Burgess (2025) observed two adults who had previously received intravenous BPC-157 through two infusion days and measurements on day three. They reported no adverse symptoms or clinically significant adverse changes in the collected measurements. That small, selected and brief observation provides little information about uncommon events, prolonged exposure or other routes.
The FDA assessments identify additional gaps involving peptide identity, impurities, aggregation and possible immune reactions. For the defined TB-500 fragment, the agency found no human clinical pharmacokinetic, pharmacodynamic or safety study. Sparse safety reporting cannot determine the frequency of harms. Neither compound has an FDA-approved medicinal product.
The direct rat experiment improves the comparison by giving the groups a common injury and assessment. It still leaves major clinical questions open: whether either preparation improves validated function, repair integrity or return to activity in people; whether any effect persists through later remodeling; and how harms compare. Combining the compounds also leaves questions about the exact preparations and how much of each reaches the injured tissue over time. The direct rat study did not establish that the combination made repaired tendons withstand more pulling force than either preparation alone.
Frequently asked questions
Is TB-500 the same as full-length thymosin beta-4?
The chemically defined TB-500 fragment is the acetylated seven-residue sequence Ac-LKKTETQ. Full-length thymosin beta-4 has 43 residues; unacetylated LKKTETQ is also distinct. A paper’s preparation determines which evidence applies.
Why can the 2026 study show significance for TB-500 without proving it beats BPC-157?
Researchers tested how much pulling force the repaired rat tendons could withstand before breaking. Tendons from the TB-500 group withstood more force than those from rats given neither peptide, and that difference met the study's statistical threshold. The BPC-157 comparison with the group given neither peptide was less certain. To decide whether one peptide worked better, however, the study had to compare the two peptide groups directly; that comparison did not establish a difference. With only four tendons tested per group, this also does not establish that the peptides work equally well.
Does the knee series show repaired cartilage or tendon?
It recorded recalled pain improvement after treatment, with no randomized control or standardized functional instrument. It did not establish structural repair by follow-up imaging.
